A form for reporting unusual incidents at your bank has been added to the WBA Best Practices Library. This free resource was developed by a WBA Financial Crimes Committee member who chose to share it with Wisconsin’s banking industry. The form has a variety of uses but ultimately is sent to the BSA Officer as a reporting tool.
The form can be used for reporting Consumer Complaints, Red Flag & ID Theft, and Suspicious or Fraudulent Activity in addition to any policy/procedure violations. It prompts staff to describe the incident in detail as to what caused them to believe something was suspicious.
All the details are collected within this report and then sent to the BSA Officer or the equivalent within the bank.
